place)
O most merciful Heart of Jesus, I beg You to grant to (Name
of the person) whom You love, to unite his/her
to Yours as You suffered our agony in the garden. Wash away
with Your most precious Blood all the sins he/she has committed
and for which he/she fears there may still be guilt.
Make his/her sufferings meritorious unto salvation. He/she
wishes to pray in this manner: O Almighty God, I give You my
body and soul for time and eternity. Forgive me all the sins
that I have ever committed. I am sorry, I love You, I believe
that Your divine Son became man and died for my sins. I believe
that there are three persons in one God, Father, Son and Holy
Spirit. I trust in You Lord Jesus to Save me. I want Your blessed
Mother, Mary to pray for me now and at the hour of my death.
I ask the prayers of my guardian angel and saints, and especially
of my patron saint. Lord Jesus, grant me eternal salvation that
I may be with You forever in the eternal bliss of my heavenly
home. Amen.

Write this: before I come as the just
Judge, I am coming first as the King of Mercy. Before the day
of justice arrives, there will be given to people a sign in
the heaven of this sort:
All light in the heaven will be extinguished, and there
will be great darkness over the whole earth. Then the sign of
the cross will be seen in the sky, and from the openings where
the hand and the feet of Savior were nailed will come forth
great lights which will light up the earth for a period of time.
This will take place shortly before the last day.
